1. Inclusion of information relating to certain questionnaire on forms to apply for disability and pension benefits under the laws administered by the Secretary of Veterans Affairs Section 5101 of title 38, United States Code, is amended— in subsection (d), by adding at the end the following new paragraph: The Secretary shall include on any covered form information relating to such disability benefit questionnaire forms, including— a statement that a veteran may choose the non-Department medical provider that completes such disability benefit questionnaire form; a statement that a veteran may submit to the Secretary such disability benefit questionnaire form at any time during the period beginning on the date on which the veteran files a claim for disability benefits under the laws administered by the Secretary and ending on the date on which the Secretary approves or denies such claim; and a description of methods by which a veteran may access such disability benefit questionnaire form, including methods to access such form without using an internet website. in subsection (e)— by redesignating paragraphs (1) and (2) as paragraphs (2) and (3), respectively; and by inserting before paragraph (2) (as so redesignated) the following new paragraph: The term covered form means— a form to apply for benefits under chapter 11 or 15 of this title; and Department of Veterans Affairs Form 21–0966 (or any successor document). The Secretary of Veterans affairs shall include on the website of the Department of Veterans Affairs titled How to file a VA disability claim (or any successor website) the information relating to disability benefit questionnaire forms described in paragraph (4) of subsection (d) of such section (as added by subsection (a)). (4)The Secretary shall include on any covered form information relating to such disability benefit questionnaire forms, including— (A)a statement that a veteran may choose the non-Department medical provider that completes such disability benefit questionnaire form;
(B)a statement that a veteran may submit to the Secretary such disability benefit questionnaire form at any time during the period beginning on the date on which the veteran files a claim for disability benefits under the laws administered by the Secretary and ending on the date on which the Secretary approves or denies such claim; and (C)a description of methods by which a veteran may access such disability benefit questionnaire form, including methods to access such form without using an internet website.; and (1)The term covered form means—
